fix(cli): use explicit global briefing fallback - #63
Reworks #63 according to the maintainer decision in #58:
ctx brief and ctx brief --hook now fall back through one server-side search for agent-briefing blocks tagged briefing:global.
Untagged blocks, topic-only tags and blocks tagged only for another project are not injected. A block may carry both the marker and project paths; a project match wins.
Project lookup errors remain distinct from legitimate empty results in the search helper and still reach the global fallback. If both requests fail, the hook degrades silently as before.
Both lookup paths use the same limit normalization: the API default for non-positive values and a maximum of 50.
The Long text, docs/operations.md, README.md and docs/ctx-as-agent-memory.md describe the producer-set marker and the follow-up boundary.
The previous client-side untagged pagination, its 500-row window and its request asymmetry are removed. No API or producer migration is included; owners tag existing general briefings once as documented in #58.
RED before implementation: the new tests failed to build because briefGlobalTag and the (result, error) helper contract did not exist.
GREEN: go test -short -race -count=1 -run 'TestBrief|TestResolveProject' ./internal/cli/ passed.
Negative controls: isolated mutations made the marker fallback and error/empty separation tests fail. The implementation was restored byte-identically and the focused suite passed again.
go test -short -race -count=20 -run 'TestBrief|TestResolveProject' ./internal/cli/ passed in a full clone with the exact staged diff.
go test -short -race -count=1 -timeout=10m ./... exited 0 in the same full clone.
Handler-aligned fakes cover API error envelopes, transport failure, malformed JSON, compact:false, tag overlap and the shared limit ceiling; a recording transport separately asserts outgoing limit 50 and default 10 on both paths.
golangci-lint run --timeout=5m: 0 issues; go vet -tags=integration ./internal/cli/ and go build ./... passed.
Pinned deadcode v0.52.0: bash deadcode.sh and bash deadcode.sh testonly passed.
Windows/amd64 and darwin/arm64: go build ./... and go vet ./... passed with CGO_ENABLED=0.
Static scan over added lines: no hardcoded secrets, shell injection, dangerous eval/exec, unsafe deserialization, SQL string formatting or merge markers.
Not run locally: Docker-backed integration shards; CI remains authoritative for those. No live /api/search call or manual Claude Code hook run was performed.

Behavior Changes
ctx brief --hook now uses only blocks tagged briefing:global when the project-specific search returns no results or fails. Project-matched blocks take precedence; untagged and differently tagged blocks are excluded from fallback results.
Search limits default to 10 when unspecified or nonpositive and are capped at 50.
Documentation
Updated the command and subagent guidance to describe the global-tag fallback behavior and search limits.
`ctx brief` (and `ctx brief --hook`, the SubagentStart hook) is documented
to fall back to untagged agent-briefing blocks when none carries the
project tag: the command's Long text and the comment above the fallback
both say so. The code searched the agent-briefing category without any
tag filter instead, so a subagent started in a project without its own
briefing blocks was handed the briefing blocks of other projects.
This takes the first option in #58: the documented untagged-only
fallback. Both sentences and the fallback call arrived in the same
commit (2a6c8a2, "feat(cli): ctx brief — project-scoped agent
briefing"), whose body describes the command as scoped by project tag.
The search API has no "no tags" filter, but every result carries its
tags, so the CLI now filters the fallback itself: it browses the
category 50 rows per page (the REST cap), follows the next_after
cursor, keeps the blocks with no tag, and stops at --limit blocks, at
the last page, or after 10 pages. Filtering a single limit-N page would
starve the fallback whenever other projects' blocks are the most
recently updated ones. With no project to scope by (the working
directory cannot be resolved), the result is the untagged set as well,
not every readable briefing block. Both output modes share this block
selection.
The Long text now says what "untagged" means and that other projects'
blocks are never included; docs/operations.md describes the fallback
next to the hook setup.
Fixes#58
The CLI now searches project-tagged briefing blocks first, then uses blocks tagged briefing:global when the project search returns no blocks. Search limits default to 10 and are capped at 50. Documentation and tests describe and cover the fallback behavior.

Merge Risk: Minimal · up to 933de
The global-tag fallback matches the server’s search behavior. No actionable merge-blocking risk remains after normal checks.
Security architecture risk: Minimal · up to 933de
The fallback now requires an explicit global marker instead of searching all readable briefings. Existing authentication and scope restrictions remain in force. No material security risk introduced or worsened by this change was identified.

No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — Fallback can place globally marked content from different projects into the receiving agent's context when the configured key already has visibility through read scopes or explicit grants. The marker does not create cross-scope access or confer additional tool privileges.
Security Findings and Attack Paths
inferred — A principal able to author an eligible, readable briefing can influence the agent's supplied context. That content-author influence predates this PR: the baseline forwarded results from a fallback with no tag restriction. The new explicit marker does not establish a newly introduced authorization bypass or prompt-injection finding.
Trust Boundaries and Controls
observed — Authentication and row visibility remain server-side controls, independent of client-selected project or global tags. The inspected search path rejects missing authentication and forwards authenticated scopes and resolved grants into storage filtering. Producer identity is not validated by the marker predicate itself.

Explanation
For #58, the PR selects the explicit briefing:global fallback. go/internal/cli/brief.go applies the marker after an empty or failed project lookup. The implementation excludes untagged and differently tagged blocks, while project matches retain precedence. brief_fallback_test.go covers marker filtering, precedence, empty results, lookup errors, and both limit paths. README.md, docs/ctx-as-agent-memory.md, and docs/operations.md document the selected behavior.

Review — fix(cli): ctx brief falls back to untagged briefing blocks only (#63)

CI on the head: 21 checks, 15 pass, 6 skipping, 0 fail (two readings 90 s apart). Read:go/internal/cli/brief.go and brief_fallback_test.go end to end on base and head; the /api/search path end to end (handler/context_search.go, handler/search_core.go, store/blocks.goSearchBlocks/searchBlocksSQL/runSearchBlocks, cli/client.go); handler/ratelimit.go, handler/middleware.go (request log); consumers cli/persist.go, cli/init.go, cli/commands.go; the Hermes plugin's write paths into agent-briefing (integrations/hermes/plugin/ctx_checkpoint/memory/{config,provider,cli,tools}.py); internal/provenance/provenance.go (.git file handling). Not read: the MCP search transport beyond the field-parity test, web/, Claude Code's hook runner. Body vs head: matches. The body names 80251b8 with parent 6b62b18; 1 commit, 3 files, +315/−15 (not stated in the body; API and local numstat agree). Sampled claims hold: the base comment at brief.go:96, the subject of 2a6c8a2, the REST cap of 50 (handler/context_search.go:150-151), next_after (:255-257), red/green, and "64 packages ok" (65 packages with tests, minus TestResolveProject in a .git-file worktree). One imprecision: config.py:62,65 show the category; the source:/profile:/hook: tags come from config.py:328-329. The merge-order note covers "#55 first" only (§2, §3.8).
1. What the change does
As stated, the PR takes the first option of #58: when no agent-briefing block carries the project tag, ctx brief and ctx brief --hook fall back to the blocks with no tag at all. The diff does exactly that: runBrief asks for project-tagged blocks only when a project resolves, then calls the new searchUntaggedBriefBlocks, which browses the category through /api/search 50 rows per page (compact:false), passes next_after back verbatim as after, keeps rows with len(tags) == 0, and stops at --limit blocks, the last page, or 10 pages (500 rows). searchBriefPage is extracted as the shared request; briefBlock gains Tags, briefResponse gains NextAfter. The Long text and docs/operations.md:88 describe the fallback and the 500-row window; five httptest tests pin it.
Where intent and effect part: the code filters on "no tag", but #58's observable problem ("A subagent started in a project without its own briefing blocks gets another project's briefing injected into its context.") is only closed if "untagged" means "project-neutral", and nothing in the repository establishes that (§3.1). The fallback also inherits resolveProject's .git-directory-only rule, which now decides whether a worktree sees its own briefing at all (§3.3).
2. Overlap and duplicates
Search scope: gh search prs --repo GottZ/ctx without a state filter (open, closed, merged; title, body, comments) for 16 terms (searchUntaggedBriefBlocks, searchBriefBlocks, runBrief, next_after, agent-briefing, "brief --hook", brief, briefing, untagged, fallback, 58, brief.go, operations.md, SubagentStart, "project tag", briefPageSize); a local title and body search over all 34 PRs of the repository (17 terms, adding briefMaxPages, "ctx brief", "#58"); the path intersection of the 3 changed files with all 34 PRs; the cross-references on #58 (only #55 and #63); git merge-tree against the heads of #62, #55 and #57; go vet and go test -short for internal/cli on the 63+62 tree. Not searched: diff contents of closed PRs beyond their path lists, forks, other repositories.

Before → after: On the maintainer's live instance the key reads 7 agent-briefing blocks: 2 untagged, 1 tagged with a repository path, 4 tagged with topics only. Base: 7 blocks, 17 430 B of hook JSON. Head: 2 blocks, 929 B, namely the two untagged ones, and both are briefings of other projects by content (they begin Project: <name>. Stack: …). For the repository that has a path-tagged briefing, base and head return the same single block.
Evidence: Base and head binaries against the live ctxd in three independent runs (7 → 2 blocks; 17 027 → 827 B human output; 17 430 → 929 B hook JSON); a SQL count on the store; an httptest probe with the same seven rows on both commits (base 7/7/1, head 2/2/1 for foreign project / hook with a foreign cwd / own repository). The Long text is literally true, since these blocks carry no tag, but #58's effect remains for every untagged project briefing. No producer in the repository writes untagged blocks as "general": ctx init writes no briefing, the Hermes plugin always tags (config.py:328-329), ctx persist tags with the project (persist.go:92,99). An untagged block comes from ctx save without --tag (commands.go:268-275 sends "tags": [], and the upsert sets tags = EXCLUDED.tags, store/blocks.go:569), that is, by omission.
Who pays / who is exposed: Every subagent start in a repository without its own path-tagged briefing, on a key that can read any untagged project briefing. At the target (several projects per key, a hook on every subagent start) the untagged pool grows with every project whose briefing was saved without a path tag. Measured: the injected text shrinks from 17 KB to 0.9 KB on the live instance; what remains is still another project's context.
Fix: Make "project-neutral" a property the producer sets instead of inferring it from the absence of tags: reserve a tag (for example briefing:global) and run the fallback as one request with tags: ["briefing:global"]. That needs no API change (the existing overlap filter tags && $n, store/blocks.go:1444), no client paging and no window, and it also settles §3.2, §3.4 and §3.5. Existing general briefings are tagged once. If "untagged = project-neutral" stays the rule, please say so where users read it: the Long text and docs/operations.md:88 should state that untagged blocks are treated as project-neutral and are injected into every project without its own briefing, and the body should not read as if the injection of other projects' briefings were gone. Test to pin either way: a fixture with an untagged block whose content is Project: other. … and a project miss, asserted as TestBriefFallbackRequiresGlobalTag (0 blocks unless marked) or as TestBriefFallbackReturnsUntaggedRegardlessOfContent (the documented convention).
Medium — The fallback is a client-side filter over a paginated list: beyond the 500 newest rows it returns nothing, silently, after 11 requests [executed]
Entry point: The same hook in a project without its own briefing, once the key reads 500 or more tagged agent-briefing rows that are newer than the newest untagged one.
Before → after: httptest fake modelled on the handler (cap 50, next_after on a full page, ORDER BY updated_at DESC, id DESC): 500 newer rows tagged source:hermes, profile:default, hook:session_end plus one older untagged briefing. Base: 2 requests, 20 foreign digests (the #58 bug). Head: 11 requests, 0 B on stdout, 0 B on stderr, exit 0, in both modes. With 499 tagged rows the head returns the untagged briefing and the base does not (its window is the 20 newest rows). So the head widens the reach from 20 to 500 rows and turns a wrong answer into an empty one. That is an improvement over the base, with a finite horizon and no diagnostic.
Evidence: Two independent probes on both commits. brief.go has no stderr or log path; TestBriefFallbackPageCap (brief_fallback_test.go:212) asserts the empty output, so the silence is pinned as correct. The repository states a rule for this pattern, at handler/context_search.go:53-56 ("opt-in SERVER-side type filters — bind parameters in the store layer, never a client filter over paginated lists", comment lines joined) and at store/blocks.go:1339-1341 ("A client-side filter over paginated lists would be functionally wrong at the 1M+/10k-issues target scale"). Both are written for the type filter; the mechanism here is the same. The in-repo producer that fills the category is the Hermes plugin: session digests and the MEMORY.md mirror go to agent-briefing (config.py:62,65), always tagged (provider.py:673), one new block per segment (provider.py:1230).
Who pays / who is exposed: An operator whose key sees Hermes digests (or many projects' tagged briefings) next to Claude Code hooks in the same scope. Once more than 500 tagged rows are newer than the general briefings, every subagent start in a project without its own briefing gets no briefing, and nothing reports it. The growth rate is not measured (0 Hermes rows on the live instance); with the plugin defaults (every 40 exchanges, every 6 h, idle 360 min, session end; config.py:77-79) it is a matter of months per active profile (this rate is an estimate, not a measurement).
Fix: The server-side form the body rejects for parity is what the repository rule asks for: an optional untagged field on searchRequest (handler/context_search.go:42) and searchInput (handler/mcp.go:111) that adds cardinality(tags) = 0 next to tags && $n in searchBlocksSQL, with the field added to the list in TestSearchTransportsAgreeOnTheAliasUnion (search_core.go:31-40 already documents one deliberate REST/MCP difference). Or the marker tag from §3.1, which needs no API change. Either removes the loop, both constants and the window sentence in the docs. If the loop stays, the smallest change is a signal: when the loop ends on the page cap with next_after still set and fewer than limit blocks, write one stderr line (for example ctx brief: no untagged agent-briefing block among the 500 most recently updated); stderr leaves the hook JSON on stdout untouched. Test to pin: extend TestBriefFallbackPageCap with stderr capture (stdout empty, 1 + briefMaxPages requests, stderr mentions 500), and assert an empty stderr in TestBriefNoProjectUsesUntagged.
Medium — A worktree outside its main repository loses its own project briefing [executed]
Entry point: The hook with stdin {"cwd":"<sibling>/go"}, where <sibling> was created with git worktree add ../<sibling> and its .git is a file (gitdir: …).
Before → after:resolveProject skips the .git file (info.IsDir()) and returns the cwd, so the project-tag search finds nothing. Base: the unfiltered fallback included the main repository's path-tagged briefing, together with everything else (live: 7 blocks). Head: untagged blocks only (live: the 2 foreign ones); the repository's own briefing is gone. Nested worktrees (verified: <repo>/.wt/<name>) are not affected, because the walk reaches the main repository's .git directory.
Evidence: A Go probe on both commits that feeds stdin through a pipe into runBrief against an httptest fake, with the layout outside any repository: sibling worktree, base own=true other=true general=true, head own=false other=false general=true; nested worktree own=true on both. Live with base and head binaries against ctxd: sibling layout, base 7 blocks including the repository's briefing, head 2 untagged blocks. No test covers a .git file: brief_test.go:81-103 uses a .git directory, the new tests pass project explicitly (brief_fallback_test.go:134,154,179,224), and no test references projectFromStdin. The body's own verification notes the same root cause for TestResolveProject ("resolveProject only accepts a .git directory").
Who pays / who is exposed: Anyone running Claude Code with the SubagentStart hook in a sibling worktree or another .git-file checkout: every subagent starts without the project's briefing, silently (brief.go:101, :121-123). ctx persist --hook resolves its tag with the same function (persist.go:92), so blocks persisted from such a checkout carry the checkout path rather than the repository root. How many hook users work in sibling worktrees is not measured.
Fix: Let resolveProject follow a .git file the way internal/provenance/provenance.go already does (:88-107 reads the gitdir: line and resolves relative paths, :133-143 reads commondir): linked worktree → <main>/.git/worktrees/<name> → commondir → <main>/.git → project = its parent directory; for a submodule (no commondir) the choice is yours. Update docs/operations.md:88 ("a .git directory or worktree file"). This also corrects the tag in persist.go:92 and makes TestResolveProject layout-independent. It can land here or as a tracked follow-up; in the second case please do not fix the ".git directory" rule into the docs now. Test to pin: TestResolveProjectLinkedWorktree with a layout under a directory that has no .git ancestor (note that t.TempDir() follows GOTMPDIR, which may sit inside a repository): main/.git/worktrees/feat/commondir = ../.., main-feature/.git = gitdir: <main>/.git/worktrees/feat, expecting resolveProject(main-feature/go) == main; plus a hook-path case through stdin that expects the main-tagged block.
Low — The fallback costs up to 11 sequential full-content requests, each a scan of the whole category; with fewer untagged blocks than --limit that is the normal case [executed]
Entry point: Every hook run in a project without its own briefing.
Before → after: Base: at most 2 requests (one page of --limit). Head: 1 + min(ceil(N/50), 10) requests with compact:false, and the loop exits early only after limit (default 20) untagged blocks (:162). Probe in hook mode with limit 20: 2 untagged + 600 tagged rows, base 2 requests / 42 781 B, head 11 / 1 067 149 B; 2 untagged + 100 tagged, head 4 / 217 773 B; 20 untagged + 600 tagged, head 2. Live today (7 rows) both commits make 2 requests; the head hook takes 0.04–0.07 s end to end.
Evidence: Live /api/search walks of 10 pages, compact:false, limit 50 over next_after on two larger categories: 0.28–0.38 s and 1.57–3.71 MB per walk, nearly all of it content of rows the client drops. A compact:true page took 27 ms / 33 KB and still carries tags. EXPLAIN ANALYZE: only idx_context_category exists (go/migrations/113_baseline.sql:234), no (category, updated_at, id) index; a cursor page reads every row of the category ("Rows Removed by Filter: 9900" at 9 900 rows, about 9.8 ms per page); on a bench DB with 1 060 000 rows in one category about 335 ms for page 1 and about 840 ms per cursor page. /api/search consumes no read rate-limit budget (it checks the query bucket but writes no context_access_log row).
Who pays / who is exposed: Every subagent start in a project without its own briefing once more than 50 tagged rows precede fewer than 20 untagged ones. At the target (many projects per key with one tagged briefing each, few general ones) that is every such start: about +0.3–0.4 s and 1–4 MB per start on today's row sizes, and 10 category scans in the database.
Fix: The server-side filter or marker tag from §3.1/§3.2 makes this one request. If the loop stays: browse with compact:true (tags are included) and fetch the full content only for the kept IDs; optionally an index on (category, updated_at DESC, id DESC) WHERE NOT is_archived. Test to pin: a fake that records compact and response bytes; "2 newest untagged + 600 tagged, limit 20" asserts that every scan request sends compact:true (or, with a server filter, exactly 2 requests).
Low — --limit above 50 has a different ceiling per path: 50 with project blocks, up to 500 in the fallback [executed]
Category: correctness
Where:go/internal/cli/brief.go:147-149, :162; the project path sends limit once (:135) and the server caps it at 50 (handler/context_search.go:149-152).
Entry point:ctx brief --limit 100, or a hook command with --limit 100, in a project without its own briefing.
Before → after: Base: 50 on both paths. Head: project path 50; fallback 100 blocks (3 requests); --limit 1000 gives 500 blocks (11 requests). The default of 20 is unaffected.
Evidence: Probe on both commits: 120 untagged rows, limit 100 → base 50 blocks / 2 requests, head 100 / 3; 600 untagged, limit 1000 → base 50, head 500 / 11; 120 project-tagged rows, limit 100 → 50 on both. The commit and body name the 10-page/500-row bound; neither names the resulting asymmetry, and no test pins either ceiling.
Who pays / who is exposed: Operators who set --limit above 50 in the hook command: the less specific path injects up to ten times more context than the project path.
Fix: Clamp limit to briefPageSize once in runBrief, before both paths (the paging stays, to find 50 untagged blocks behind tagged ones); or document the path-dependent ceiling in the flag help and the docs. Test to pin: TestBriefLimitCapIsPathIndependent, 120 tagged versus 120 untagged rows at limit 100, both (50 blocks).
Low — An error and an empty result are the same nil: a failed project request silently serves the untagged fallback, a failed page silently truncates [executed]
Entry point: The hook while ctxd answers the project request with 500 {"success":false} (store error, handler/context_search.go:192-193), a proxy answers 502, or the request runs into the 120 s client timeout (client.go:48).
Before → after: Base: the unfiltered fallback, so the project's own block still appeared, among everything else. Head: untagged blocks only; the project's own blocks are missing and nothing says why. A failure on fallback page 2 yields "(1 blocks)" instead of 5, after 3 requests. A truncated body gives empty output on both.
Evidence: Probe with six cases on both commits: for 500 with JSON, 429 as text, 502 as HTML and timeout, head made 2 requests with mine=false general=true, base returned mine=true other=true general=true. The body says "a failed page ends the loop with what it has"; the project-request case is not mentioned, and no user-facing text describes either. /api/search itself emits no 429; that case stands for a proxy.
Who pays / who is exposed: A subagent whose start coincides with a transient ctxd failure, in a project that has its own briefing: generic instead of project context, not reproducible afterwards. No data change, no exposure (the untagged blocks are readable by the key anyway).
Fix:searchBriefPage returns (*briefResponse, error): a Post error, an unmarshal error and !Success are errors, Count == 0 is empty. After a project-request error, either do not fall back or write one stderr line (ctx brief: project search failed: …); the direction is your call, the defect is that the two cases cannot be told apart. A page error writes a stderr line with the page number. Test to pin: a fake that answers the tagged request with 500 expects 1 request and no fallback output (or the stderr line); a fake that fails page 2 expects stderr to name page 2.
Low — The httptest fake differs from the handler in three ways the code depends on; three mutations of brief.go stay green [executed]
Entry point:go test -short -run TestBrief ./internal/cli/ against a modified brief.go.
Before → after: Base: no tests for ctx brief. Head: 5 tests that do not catch the three mutations below.
Evidence: All 5 PR tests stay green for each of: M2, a cursor passed back without after_updated (the real handler then serves page 1 again, handler/context_search.go:157-160; confirmed live, a cursor with only after_id returns the ids of page 1); M3, "compact": false removed (the live default is compact:true with content_preview and no content, which yields empty briefings); M4, return nil instead of break at brief.go:154-156 (drops the partial result the body describes). A stricter fake catches all three; the unmutated head passes it.
Who pays / who is exposed: Nobody today, brief.go is correct. A later refactor of the cursor type, the compact flag or the error path would pass CI.
Fix: Decode after into store.SearchCursor (or the same shape) and apply the handler rule (empty id or zero time → page 1; keyset (updated_at, id) < cursor); honour compact with default true (content_preview only); allow request N to answer 500 {"success":false}; add a "failed page keeps the partial result" test. Two cheap additions: a row with "tags": null (the column is nullable, go/migrations/113_baseline.sql:61) and a category of exactly 50 rows (full page → second request with count 0).
Low — Merging #63 first and #55 unchanged leaves README and the agent-memory page describing the removed fallback, without a git conflict [executed]
Entry point: The maintainer merges #63, then #55 as it stands.
Before → after: On root today neither the README sentence nor docs/ctx-as-agent-memory.md exists. After both merges, in either order (identical tree 1c02f435), README.md:105 and docs/ctx-as-agent-memory.md:30 say "whatever their tags (#58)", while docs/operations.md:88, the Long text and the code say "no tag at all". ctx-as-agent-memory.md:30 links to the operations.md section that contradicts it and cites #58, which this PR closes.
Evidence:git merge-tree --write-tree in both orders → tree 1c02f435, exit 0; a real git merge --no-commit in a throwaway worktree reports "Automatic merge went well"; git grep on the merged tree finds both sentences. The body promises the update only "if #55 lands first". On #55 you wrote "Whichever way they land, that change updates these sentences."; #63 cannot do that while the sentences are not on root.
Who pays / who is exposed: Readers of the README and the agent-memory page after both merges; the maintainer, to whom git reports nothing.
Fix: Whichever PR lands second carries the two sentences (for example "the most recently updated untagged agent-briefing blocks, never blocks tagged for another project", without "(#58)"). Before the second merge, git grep -n 'whatever their tags' README.md docs/ must print nothing.
4. Declared behaviour changes (not defects)
Topic-only tagged briefings leave the fallback. Declared in the body: "A briefing tagged only with a topic (not a project path) is no longer in the fallback." On the live instance the four topic-tagged briefings (about 15.5k characters) no longer reach subagents in the other repositories on that host (base 7 blocks / 16 981 B of context, head 2 / 801 B). On the base they reached any repository only because the base injected every readable briefing everywhere. The same applies to tags of the form project:<name>: a user-local init command template outside this repository tags briefings that way, which never matches the path resolveProject produces; the base delivered such a block only through the unfiltered fallback (to every project), the head delivers it nowhere (probe: base injected, head not). The remedy is on the data side: tag such blocks with the repository path(s); the overlap filter allows several. Please confirm in #58 that this is intended, and consider pinning it with a pure topic tag (tags: ["control"]) and a project:mine tag in TestBriefFallbackIsUntaggedOnly.
Hermes-tagged blocks leave the fallback: the session digests and the MEMORY.md mirror. The body cites config.py:62,65 but calls both "Hermes session digests". Line 62 is CATEGORY_MEMORY (the MEMORY.md mirror, written via on_memory_write, provider.py:737-763), excluded the same way through merge_tags (provider.py:673). Probe: base injects a mirror block, head does not. Live there are no such blocks yet. Please name the mirror in the body and in docs/operations.md:88, for example "blocks with the plugin's provenance tags (source:, profile:, hook:), the digests and the MEMORY.md mirror, are never part of the fallback".
Without a resolvable project the result is the untagged set, not every readable briefing, and no request with tags: [""] is sent any more. From the CLI this branch is reachable only when os.Getwd fails (projectFromCWD, brief.go:214-219).
Worst case 11 sequential requests instead of 2 per SubagentStart (declared). §3.4 covers how often the worst case occurs.
A 500-row fallback window, documented in docs/operations.md:88. §3.2 covers what happens beyond it.
5. Checked and found in order
Paging contract of /api/search for the category browse: after is decoded (handler/context_search.go:52), a null or empty cursor means page 1 (:157-160), limit default 10 (search_core.go:126-131) and cap 50 (:149-152), next_after only on a full page without a query (:203, :267-280); the store keyset (updated_at, id) < (…) applies only without a query (store/blocks.go:1486-1490) under ORDER BY updated_at DESC, id DESC (:1517, :1530), a total order. No post-filter sits between SQL and cursor (runSearchBlocks, :1565-1566), so a short page really is the last one. Live: 694 rows over 14 pages without duplicates, 23 updated_at ties resolved correctly, page edges placed on a tie of three rows; agent-briefing with limit 2 (2/2/2/1, id-identical to a single 50-row page) and limit 3 (3/3/1, no duplicates); an exactly-full last page is followed by count 0 and next_after: null, which the CLI ends on (brief.go:196).
The CLI passes the cursor back as json.RawMessage without re-encoding (brief.go:166-169, :186-188; byte-identical in the probe), and recognises both a JSON null and a missing key as the end (:166).
Every result carries tags (BlockPreview.Tags without omitempty, store/blocks.go:176); a NULL column arrives as JSON null and counts as untagged. Live: 0 rows with NULL tags, 0 with NULL updated_at.
limit <= 0 → 10 matches the handler default and is needed for the len(out) == limit exit (brief.go:147-149).
The project-scoped path is unchanged in effect: overlap filter tags && $n, server cap 50; for a repository with a path-tagged briefing base and head return the same block live, including from nested worktrees.
Trust boundary: same endpoint, key and scope/grant clause (store/blocks.go:1499, handler/search_core.go:194-200); the client filter only narrows the result.
Rate limit: /api/search checks the query bucket (handler/context_search.go:123-124) but writes no context_access_log row, so the extra pages consume no read budget; the read limit is 0 by default (config.go:866) and off live.
Logging: brief.go has no stderr or log path; the ctxd request log (handler/middleware.go:185) records method, path, status and duration only, no tags, project path or body.
Response size: the 10 MB client cap (cli/client.go:14) is far above live 50-row pages (at most 404 KB measured).
Error path: a failed request ends the loop (brief.go:154-156), no retry amplification; on a server outage the hook stays at 2 requests as on the base.
Callers: runBrief, searchBriefBlocks, searchUntaggedBriefBlocks and searchBriefPage are used only in brief.go and its tests; resolveProject is also used by persist.go:92 (unchanged); ctx init writes only the hook command (init.go:852); the Hermes plugin does not call ctx brief.
TestResolveProject (not in the diff) fails in .git-file layouts and passes with a .git directory, identically on base and head (git archive exports outside any repository); resolveProject is byte-identical, only moved from line 158 to 224. The body's statement is accurate; CI checks out a .git directory.
Red/green as in the body; -count=20 without flakes; captureStdout swaps os.Stdout globally but no test in internal/cli runs in parallel; TestBriefFallbackPagesPastTaggedBlocks does reproduce the starvation of the rejected one-page variant.
docs/operations.md:88 matches the code (500 = briefMaxPages × briefPageSize, the .git-directory rule at brief.go:227).
for range briefMaxPages is valid with go 1.27.0 in go.mod.
Toolchain on the head: build, vet, lint, the full short suite, both deadcode gates, both cross builds and gofmt as listed in the header.
Commit form: one commit, author and committer identical, SSH-signed, GitHub reports verified=true; subject equals the PR title in type(scope): summary form; Fixes #58 in the body.
#62 and #57 merge cleanly with this PR; internal/cli vets and tests on the 63+62 tree.
6. Existing review coverage
coderabbitai[bot], review 5744389475, 2026-11-02T22:34:13Z, on this head: the review body holds only the autofix checkbox.
coderabbitai[bot], inline comment 4448599472 on brief.go:72-74 (Minor), #63 (comment): "Narrow the 'another project' statement". Through the overlap filter (store/blocks.go:1444) a block tagged [X, Y] is returned for project X; the suggestion is "tagged only for another project". On this head: open (unresolved, not outdated, no reply, no later commit). It holds as read and concerns wording only; docs/operations.md:88 phrases it in the fallback context and is not affected. If §3.1 changes the Long text, both fit one edit.
coderabbitai[bot], summary comment 6589724135 (created 22:09:55Z, updated 22:39:41Z), #63 (comment): one finding (the inline above), merge risk "Low", pre-merge checks 5/5 passed (docstring coverage 80.00 % at the 80.00 % threshold), and the observation that the 500-row cap can produce an empty briefing. The author documents the cap in docs/operations.md:88; §3.2 adds the measurement and the missing signal.
Human reviews: none yet; reviews were requested from GottZ and W4-NERF.
New in this review: §3.1 and §3.3 to §3.8, the measured extent of §3.2, the MEMORY.md mirror and the project:<name> case in §4.
7. Verdict
Request changes. The code does what the body says and every gate we ran is green, but on real data the fallback still injects other projects' briefings (§3.1), its client-side window ends without a signal (§3.2), and worktrees outside the main repository lose their own briefing (§3.3). Open gates: the integration shards were not run locally (green in CI), there was no live run with more than 500 agent-briefing rows, and the maintainer's decision on #58 is pending.
The fallback key is the maintainer decision #58 left open, and §3.1 shows that the data does not support "untagged = project-neutral" by itself. I will settle the direction in #58 before you rework, so that this PR does not chase a moving target. The two candidates on the table: a producer-set marker tag such as briefing:global, queried through the existing server-side tags filter (one request, no client paging, no window; settles §3.1, §3.2 and §3.4 and makes §3.5 moot), or keeping "untagged = project-neutral" as a stated convention (then: say so in the Long text, docs/operations.md:88 and the body, and signal an exhausted window on stderr, §3.2).
Let resolveProject follow a .git file through gitdir/commondir (§3.3), here or as a tracked follow-up; either way do not document the .git-directory rule as the contract.
Separate errors from empty results in searchBriefPage (§3.6), and clamp --limit to 50 on both paths (§3.5).
Align the test fake with the handler (cursor rule, compact default, error responses) and add the failed-page test (§3.7).
Let whichever of #55 and #63 lands second carry the two #55 sentences (§3.8), and take CodeRabbit's "tagged only for another project" wording in the same Long-text edit.
8. Not verified / questions for the author
The integration shards (handler, store, guard, heavy, distill, rest) were not run locally; they are green in CI on this head. store/keyset_search_integration_test.go was read, not run.
No live run with more than 500 agent-briefing rows (the live instance has 7). The window and cost results come from httptest fakes on both commits and from live paging on other categories.
Claude Code's SubagentStart hook timeout, and whether hook stderr is shown on exit 0, were not checked. They decide how long a degraded ctxd can hold a subagent start (120 s per request, no overall deadline, client.go:48) and whether the stderr signal suggested in §3.2 reaches anyone.
Only one key (home scope private) of one installation was examined; other tenants and keys were not.
The Hermes digest growth rate on an installation with the plugin active is not measured (0 Hermes rows live).
Whether Claude Code creates its own worktrees nested or as siblings was not checked; it decides how many hook users §3.3 reaches.
[hypothesis] With 50 rows per page, a page of blocks averaging more than about 200 kB exceeds the 10 MB client cap and the walk ends with empty output (a fake with 50 × 250 kB rows did so on the head, while the base with 20 rows stayed under the cap). Live relevance: the largest live block has 42 913 characters and the store accepts at most 1 MB per block. The check that would settle it: the largest agent-briefing block size across real installations.
[hypothesis] A block whose updated_at rises between two fallback pages moves ahead of the cursor and is skipped in that walk. This is inherent to keyset paging and was not probed; a fake with the row-value keyset that updates a row between page 1 and page 2 would settle it.
Question: which command or document is meant to create the untagged "general" briefings that option 1 relies on? Nothing in the repository writes them today (§3.1).
…top_grace_period (#71)
Wave W06-1 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §7 block A). No `/v1` surface, no migration.
## What this changes
The `context_llm_log` write path started one goroutine and one pool
connection per row, logged failures at DEBUG only, and ctxd closed its
pool on shutdown without waiting for pending inserts; the `ctx` compose
service had no `stop_grace_period`, so Docker's 10 s default could
SIGKILL the daemon inside its 15 s HTTP drain.
- `internal/llmlog`: per-pool bounded writer queue — at most 4 inserts
run at once, at most 4 096 rows wait; beyond that `Record` drops and
counts the row. Failed inserts are counted; one WARN per minute at most,
carrying the totals since start (the per-failure DEBUG line stays). New
readers `InsertFailures()` / `InsertDropped()`. `Record` stays
non-blocking; `Settle` now also waits for queued rows that have not
started.
- `cmd/ctxd`: the teardown is `shutdownSequence`: `srv.Shutdown` (15 s)
→ `llmlog.Settle` (5 s) → `scheduler.Wait` → `llmlog.Settle` (5 s) →
pool close. Settling twice is deliberate: a running dream cycle does not
stop on SIGTERM (own context, `dream.cycle_timeout`, 700 s by default),
so a settle only after `Wait` would hand the HTTP rows to the SIGKILL
that ends an over-long wait.
- `internal/handler`: server-admin-only `/api/status` section `"llmlog":
{"insert_failures", "insert_dropped"}`, present at 0/0, absent on the
tenant path and on the SSE frame.
- `docker-compose.yml`: `stop_grace_period: 30s` on `ctx`, pinned ≥ 25 s
by `internal/config` `TestComposeCtxStopGracePeriod` (scanner follows
the top-level section, so only `services.ctx` counts).
- `docs/operations.md`: shutdown / write-path section and an upgrading
note for operators with their own compose file; `docs/api.md`: the new
status section.
## Behaviour changes (intended)
- A burst of LLM calls no longer takes the whole 20-connection pool for
its telemetry rows (before: 20 of 20 under a 200-row burst; now 4).
- Rows beyond 4 096 queued are dropped and counted instead of queued
without bound.
- Shutdown waits up to 2 × 5 s longer; rows still pending after that are
lost and logged as such.
- Operators running ctx from their own compose file should set
`stop_grace_period: 30s` (upgrading note).

Searched open PRs #55/#57/#63/#67 for shared paths on 2026-11-03: none
(`docs/operations.md` is touched by #63 in a different section —
whichever lands second rebases trivially). #69 (merged `16fe6436`)
touched ingest only; this branch is rebased on it.
## Cost
Per LLM call: one mutex lock on enqueue instead of one goroutine spawn;
inserts run on ≤ 4 connections instead of ≤ 20. Measured by the
reviewer's throughput probe: 4 096 real rows in 327 ms (0.32 ms per row
and slot), so a 5 s settle covers a full queue on a healthy database;
the break-even is 4.9 ms per insert. Status: the two counters are
process memory, read per call, no I/O.
## Verification
Gates, each red on `7f17de21` first:
- (a) failure counter + throttled WARN + status field: red = compile
errors (`InsertFailures`, `LLMLog` undefined; failures were DEBUG only)
→ `DROP TABLE context_llm_log` makes `InsertFailures()` +1; 100 failures
→ exactly 1 WARN, the next after the interval with total 101;
`llmlog.insert_failures` 1 on the admin path, absent for tenants.
- (b) 200 simultaneous `Record` against a 50 ms insert delay (pgx
tracer, MaxConns 20): peak concurrent inserts 20 → 4, rows 200/200.
- (c) 50 `Record` + slow inserts, then the shutdown sequence: old order
lands 0/50; `shutdownSequence` lands 50/50 plus 10/10 written during
`scheduler.Wait`. Hardening: HTTP rows landed at `Wait` entry 0/50 →
50/50.
- (d) compose pin: red "declares no stop_grace_period" → 30s; scanner
pin 3/6 → 6/6 on the shapes that fooled the first version.
`go/deadcode-testonly-allow.txt`: the `llmlog.Settle` line is removed —
it recorded Settle as reachable from tests only, and this PR gives it
its production caller; `deadcode.sh testonly` was red on the first push
for exactly that line and is green without it (third commit).
Suites on the rebased head: `go build ./...`, `go vet -tags=integration
./...`, `golangci-lint run` with and without `--build-tags=integration`
(0), `go test -short -race ./...` (65 ok), integration `-p 1` for
`cmd/ctxd`, `internal/config`, `internal/llmlog`, `internal/handler`
(full handler suite on the first commit). Independent review by a fresh
agent: stress test with `-race` at GOMAXPROCS 1/4/16 (50 × 1 000
`Record`, 10 % injected failures, 3 concurrent `Settle` callers): landed
+ failed + dropped == sent in all three runs, `pending` 0 after
`Settle`, `running` ≤ 4, queue ≤ 4 096; lost-wakeup probe 160 000/160
000 rows, 0 settle timeouts.
## Not verified
- Frequency of shutdowns that coincide with a dream cycle longer than
the grace period on the live host (the second settle is lost in that
case; the first still lands the HTTP rows).
- `scheduler.Wait` covers the dream workers only; `runSensitivityAudit`
(`events/audit.go:124`) and the dream-loop backfill
(`scheduler.go:1832`) can still `Record` after `Wait` returns — those
rows are covered by the second settle only if they are queued before it
runs. Candidate for the W06-13 force-settle wave.
- `ctxServiceEnvValues` in `compose_surface_test.go` scans the same way
the first version of this gate did (no `services:` check); left as is,
noted for a later sweep.

…K (Migration 158) (#72)
Wave W06-2 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §3.1 M-06-1). Migration **158**, no new table, no
`/v1` surface, no reader.
## What this changes
- `go/migrations/158_llm_log_consumer.sql`: `context_llm_log.consumer_id
UUID` (nullable, no FK — like `api_key_id`, the log outlives the
credential), `CHECK ck_llm_log_one_identity (num_nonnulls(api_key_id,
consumer_id) <= 1)` on the hypertable and every chunk, partial index
`idx_llm_log_consumer (consumer_id, created_at DESC) WHERE consumer_id
IS NOT NULL`. Idempotent (`IF NOT EXISTS`, `duplicate_object`
swallowed), `lock_timeout 3s`, no backfill (every existing row is
internal).
- `internal/llmlog`: `Entry.ConsumerID` and the 24th insert parameter
(`nullUUID`). No production code sets it yet — the gateway stamp (W06-9)
will be the only setter; every row keeps `consumer_id` NULL until then.
- Schema-contract manifest regenerated (column + index; minimum
extension versions kept at 2.30.1 / 0.8.6 as 156/157 did). `test.sh` T07
unchanged (0 new tables; the column counter covers `context_blocks`
only).
- `docs/operations.md` "Migration 158" (cost, downgrade, the export now
carries `"consumer_id": null` per JSONL line), `docs/architecture.md`
LLM-log paragraph.
## Behaviour changes (intended)
- A row that names both a knowledge key and a consumer is refused by the
database (`23514`); through `llmlog.Record` such a row is lost and
counted in `llmlog.insert_failures`. No current writer can produce one.
- `ctx-llmlog-export` JSONL lines gain the key `consumer_id` (additive).

(`docs/operations.md` is touched by #63 in another section). Rebased on
#71 (W06-1); `llmlog.go` hunks sit below W06-1's writer queue and do not
overlap.
## Cost
Boot-time migration, one transaction, `context_llm_log` locked (ACCESS
EXCLUSIVE) until commit. Measured on a synthetic copy of production
shape (1 000 000 rows, 29 chunks, 976 MB heap, TimescaleDB 2.31.0;
reproducible with `CTX_M158_PROBE_ROWS=1000000 go test -tags=integration
-run TestMigration158CostProbe ./internal/llmlog/`): `ADD COLUMN` 3 ms,
`CHECK` validation 0.38–0.45 s, index build 0.43–0.74 s, whole file
through the boot runner 0.73–0.75 s warm. Cold (buffers evicted,
independent review): 1 M / 976 MB ≈ 0.9–1.25 s, 3 M / 2.9 GB ≈ 2.85 s,
i.e. ≈ 0.7–1 s per GB and scan; a 30 GB heap would extrapolate to
roughly 40–60 s of lock at boot (not measured). Live table today: 352
565 rows, 29 chunks, 318 MB heap. Under a foreign lock the migration
aborts after 3 s with `55P03`, leaves no partial state (no column, no
constraint, no index, no `_migrations` row) and the next boot retries.

Gates, each red on `83461171` first:
- (a) `Record` with `ConsumerID` → column set; red: `unknown field
ConsumerID` / raw SQL `42703`.
- (b) a row with both identities → `23514` on a new and on a
pre-existing chunk, and via `Record` +1 insert failure and no row; red:
without the CHECK block the insert succeeds, 0 of 9 chunks carry the
constraint.
- (c) existing writers keep `consumer_id` NULL; `EXPLAIN` of the
per-consumer window uses `idx_llm_log_consumer` on 5/5 chunks without
`enable_seqscan = off`; red: created_at index + seq scans.
- (d) second apply leaves the catalog byte-identical (incl.
`convalidated`, `conislocal`, `coninhcount`, `indisvalid`); red: without
the `duplicate_object` handler `42710`.
Suites on the head: `go build ./...`, `go vet -tags=integration ./...`,
`golangci-lint run` with and without `--build-tags=integration` (0), `go
test -short -race ./...` (65 ok), `deadcode.sh` + `deadcode.sh testonly`
(ok), integration `-p 1` for `internal/llmlog`, `internal/store`
(migration/schema/manifest suites incl. the T07 count gate),
`internal/schemacontract`, `cmd/ctxd` (137 passed, 2 expected skips).
Independent review by a fresh agent re-ran idempotence, chunk
inheritance (`\d+` on an old and a post-158 chunk), the `55P03` abort
under a foreign AccessShare lock and under a chunk-only lock, 8
concurrent writers during the migration (28 001/28 001 rows landed, no
deadlock), the rollback (`DROP INDEX/CONSTRAINT/COLUMN`, 5 ms, old
binary unaffected, new binary gets `42703` as documented) and the cold
scans above.
## Not verified
- The schema contract does not model CHECK constraints (pre-existing): a
`ck_llm_log_one_identity` dropped by hand would not show up on
`/api/contract`. Candidate for a later wave.
- Behaviour on a heap far beyond 3 GB is extrapolated, not measured.

… and backfill (Migration 159) (#73)
Wave W06-3 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §3.2 M-06-2). Migration **159**, one new table, no
reader yet (W06-4/W06-5 follow), no `/v1` surface.
## What this changes
- `go/migrations/159_llm_usage_rollup.sql`: `context_llm_usage_hourly` —
one row per UTC hour and dimension (`api_key_id` | `consumer_id`,
`pipeline`, `backend_name`, `model`, `model_alias`, `dispatch_class`,
`backend_locality`, `hop`) with counters (`calls`, `errors`,
`unattributed_ok`, `estimated_calls`, tokens, `duration_ms_sum/_n`,
`queue_wait_ms_sum/_n`, `cost_usd`) and three 16-bucket log2 histograms
(latency, queue wait, TTFT; design/05 §3.5 contract). Unique index with
`NULLS NOT DISTINCT`, three secondary indexes, `fillfactor 50`, per-5
000-dead-tuple autovacuum. SQL helpers `llm_usage_hist_idx` (via
`width_bucket`), `llm_usage_hist_one`, `hist_add` (one inlined
`ARRAY[…]` expression). `AFTER INSERT` row trigger
`trg_llm_usage_rollup` on `context_llm_log` that never raises
(type-guarded `metadata.gw` reads, clamps, every text dimension cut to
100 characters so no unique-index entry can reach the 2 704-byte B-tree
limit). Backfill of every existing row in the same transaction after
`CREATE TRIGGER` (overwriting, `IS DISTINCT FROM` guard ⇒ a second run
is a no-op).
- Manifest regenerated (table, four indexes, four functions, one
trigger; minimum extension versions unchanged); `test.sh` T07 58 → 59
tables.
- `docs/operations.md` "Migration 159" (why a trigger, cost, delete
semantics, isolation note, rollback) + two follow-ups from the #72
review (NOT VALID switch point corrected to ≈ 15–20 s at 10 GB; helper
comment grammar); `docs/architecture.md`.
## Behaviour changes (intended)
- Every `context_llm_log` insert now also upserts its hour's rollup row
(cost below).
- Deleting or truncating `context_llm_log` rows does not decrement the
rollup (ledger semantics; ctx deletes no log rows; `EvictBodies` is an
`UPDATE`).
- Text dimensions longer than 100 characters share a rollup row with
their 100-character prefix (today's longest is 25).
## Deviations from design/06 §3.2, each measured
- `llm_usage_hist_idx` uses `width_bucket` over the 15 bounds instead of
`ceil(log2(…))` on numeric: ≈ 0.15 µs vs ≈ 11 µs per call; equal for
every ms in [0, 1 000 000] (full comparison, 0 differences) and at the
bigint edges.
- `hist_add` is one `ARRAY[…]` expression the planner inlines; the
`unnest`/`array_agg` form pushed p99 Δ to 1.9–2.2 ms under eight writers
on one row (threshold 2 ms).
- `cost_usd NUMERIC` without typmod: `NUMERIC(18,8)` overflows (22003)
after ≈ 10 001 rows at the llmlog column maximum.
- Alias cut to 100 instead of 128 (04 R-06-2): one cut for all six text
dimensions keeps the worst-case index entry provably under 2 510 bytes
(six 4-byte-per-char texts of 128 already measured at 3 096 bytes ⇒
54000).

(W06-2, `dc9923d8`); no further root movement since.
## Cost
Boot: the backfill is one `GROUP BY` over the log inside the migration's
transaction. Synthetic copy of production shape (1 000 000 rows, 29
chunks, 976 MB heap, TimescaleDB 2.31.0; reproducible with
`CTX_M159_PROBE_ROWS=1000000 go test -tags=integration -run
TestMigration159BackfillProbe ./internal/llmlog/`): backfill 8.0–8.3 s,
whole file through the boot runner 7.5–8.3 s, 77 239 rollup rows (one
per 12.9 log rows), 108 MB with indexes. Live table (352 720 rows) ≈ 3 s
extrapolated. `CREATE TRIGGER` holds SHARE ROW EXCLUSIVE on the
hypertable until commit: concurrent inserts wait, reads do not (probed
with two sessions: an insert before `CREATE TRIGGER` is counted once by
the backfill, one after waits and is counted once by the trigger).
Per insert (`CTX_M159_LOAD_INSERTS=50000 go test -tags=integration -run
TestUsageRollupLoad ./internal/llmlog/`, 8 writers, 50 000 inserts per
side, baseline = same table without the trigger): one dimension (all
writers on one rollup row) p50 258 → 634 µs, p99 626 → 1 583 µs (Δ 0.96
ms, threshold 2 ms), HOT updates 0.918 (0.894 at fillfactor 100 — red);
64 dimensions p99 Δ 0.35 ms, HOT 0.998. The gate refuses runs below 10
000 inserts.
## Verification
Gates, each red on `dc9923d8` first (`42P01` without the migration): (a)
parity over 1 000 seeds / 17 655 rows, red without `NULLS NOT DISTINCT`
(37 deviations); (b) NULL conflict: 2 rows → 1 row with `calls = 2`; (c)
histogram: p95 from buckets within one bucket width of
`percentile_cont(0.95)`, all 30 bucket bounds exact, red with `x +
EXCLUDED.x` (42883); (d) totality over 64 malformed `metadata.gw` forms,
red with a naive cast (58/64 fail, 22P02), plus a 3 000-byte `model` and
six 128-char four-byte texts (54000 before the cut, landed and counted
after); (e) second apply leaves catalog and rollup rows (incl. `xmin`)
byte-identical; (f) `EvictBodies` leaves the rollup unchanged; (g) load
gate above; (h) manifest, T07 59, count gate; both deadcode ratchets ok.
Suites on the head: `go build ./...`, `go vet -tags=integration ./...`,
`golangci-lint run` with and without `--build-tags=integration` (0), `go
test -short -race ./...` (65 ok), `deadcode.sh` + `deadcode.sh
testonly`, integration `-p 1` for `internal/llmlog` (37 passed, 3
env-gated probes run separately), `internal/store` +
`internal/schemacontract` + `cmd/ctxd` migration/schema/manifest suites
(320 passed in store), `internal/handler`/`backends`/`chat`/`llm` status
and quota suites. Independent review by a fresh agent: 37 totality forms
(`-infinity`, year 294276, `1e400` hop, `NaN` cost, NUL, surrogates —
none raises in the trigger that the table itself accepts), full bucket
comparison over [0, 1 000 000] (0 differences), exactly-once with two
sessions around `CREATE TRIGGER`, lock inventory (`pg_locks`), load gate
re-run (p99 Δ 1.04 / 0.49 ms, HOT 0.918 / 0.998), red-proofs for (b)/(d)
and a no-trigger overlay (6 tests red).
## Not verified
- Hour-boundary crossing inside the load gate (expected update count is
now derived from the rollup rows actually created; a clock-controlled
red-proof was not built).
- Under `default_transaction_isolation = repeatable read` the upsert
raises 40001 for concurrent writers (reviewer: 1 449 of 2 000 inserts
lost); ctx runs `read committed` and sets no isolation — documented, not
changed.
- Heaps far beyond 1 M rows are extrapolated.

## Summary by CodeRabbit
* **New Features**
* Added hourly summaries of LLM usage, including call and error counts,
token totals, latency, queue wait, and cost. Existing log data is
included, and new log entries update the summaries automatically.
* **Documentation**
* Updated operational guidance with migration details, estimated locking
considerations, and downgrade steps. Clarified that deleting source log

…ur (W06-4) (#74)
Wave W06-4 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §4.7 / §4.5, masterplan K17). No migration, no
`/v1` surface.
## What this changes
- New leaf package `internal/llmusage` (imports only the `pgxdb` handle
atoms; depguard rule `llmusage-leaf` pins that): `Window(ctx, q,
Spec{Now, Span, APIKeyIDs, Locality})` reads one rolling window as
**hourly rollup from the first full UTC hour after `now − span`** plus
**the raw `context_llm_log` rows of that first, partial hour** — the
exact semantics of the raw `created_at > now() - interval` queries it
replaces, with at most one hour of raw rows per window. It also returns
the earliest relief of each figure (oldest counted row + span),
microsecond-exact.
- `backends.QuotaAccountant.spend` reads its three figures (24 h calls,
24 h external cost, 30-day rolling external cost) through
`llmusage.Window` instead of a 30-day scan over the hypertable. The
filter stays the tenant's knowledge keys (`api_key_id`): **a `/v1`
consumer row never counts here** (K17, B-06-18) — consumer traffic
cannot exhaust `daily_calls` or the cost budget and lock a tenant's
people out of `/api/query` and the web chat. Relief timestamps are
stored on the tenant state for W06-14 (no reader yet).
- Docs: `docs/multi-tenancy.md` (spend source, asymmetry),
`docs/operations.md` (Migration 159 rollback now coupled to the reader),
`docs/architecture.md`.
## Behaviour changes (intended)
- Spend figures are byte-equal to the old query for the same clock
(parity property below); the windows now end at the accountant's clock
instead of the database's `now()` (same host, same clock in production).
- Three index probes per tenant refresh (one per window) instead of one
30-day scan.
## Deviations from design/06 §4.7, with reasons
- The raw edge always ends at the **next** full hour, also when `since`
lies exactly on one: the bucket starting at `since` holds a row stamped
exactly at `since`, which the strict lower bound excludes; the design
formula counted such a row twice (90 parity deviations with it, 0 with
the fix).
- Keys are probed one by one (`LATERAL … OFFSET 0`) instead of
`api_key_id = ANY($1)`: with `= ANY` the planner preferred the
time-leading indexes (`uq_llm_usage_hourly`, the log's `created_at`
index) and read every tenant's rows in the range — under the generic
plan the whole instance's edge hour. The locality condition sits in the
aggregates' `FILTER` so both renderings share one access path. Pinned by
`TestQuotaSpendPlan` under the custom and the forced-generic plan.
- Relief is exact (oldest raw row of the oldest counted rollup hour, one
bounded lookup) rather than `bucket_start`: the bucket variant reports
relief **too early** (11:00 for a row at 11:05:00.000007); a safe
conservative variant would have been `bucket_start + 1 h`.
## Consumers of the changed contracts
| Changed symbol | Callers / readers | Covered by |
|---|---|---|
| `QuotaAccountant.spend` (signature, now fills `tenantState` incl.
relief) | `refresh` (`quota.go`) | `TestQuotaAccountant_*` (backends 80

suites |
| `NewQuotaAccountant(q quotaDB, …)` (needs `Querier` + `Rower`) |

| `context_llm_usage_hourly` (first reader) | `llmusage.Window` |
`TestWindowParity` (500 seeds vs the old raw SQL), `TestWindowBounds`

| `.golangci.yml` depguard | CI lint | `golangci-lint run` 0 |
## Overlap search
Open PRs #55/#57/#63/#67 on 2026-11-03: no shared files. Based on #73
(`43041709`); root unchanged since.
## Cost
Per policy tenant every 30 s: three statements, each ≤ 1 h of raw rows
through `idx_llm_log_apikey` plus the rollup through
`idx_llm_usage_key`; measured in `TestQuotaSpendPlan`: raw rows touched
3–5 (base: 1 441, with no upper bound on `created_at` and no rollup).
Planner edge: on a rollup of a few dozen tenants the custom plan may
still pick the time-leading `uq_llm_usage_hourly` for the 30-day window
(≈ 1.5× the probe's cost); from ~100 tenants on it moves to
`idx_llm_usage_key`. Candidate for later: `INCLUDE` columns on
`idx_llm_usage_key`, one batched statement over all tenants.
## Verification
Gates, each red on `43041709` first: (a) existing quota suites green
before and after (backends 80/83, chat 1, llm 4, handler 52); (b) parity
property `llmusage.Window` vs the old `spend` SQL over 500 random seeds
with rows around both window edges — 425 deviations without the edge
rows, 90 with the design's `ceilHour`, 0 final; (c) `EXPLAIN` under
custom and generic plan: raw rows 1 441 → 3–5, rollup via
`idx_llm_usage_key`; (d) asymmetry pin: a spend-union variant (`OR
consumer_id = ANY`) counts 5 consumer calls and the gate refuses on
`daily_calls`, the shipped code counts 0 and lets the tenant through,
the control with 5 knowledge-key calls refuses; (e) relief with a fixed
clock: compile error on base, `bucket_start` variant 11:00 vs exact
11:05:00.000007.
Suites on the head: `go build ./...`, `go vet -tags=integration ./...`,
`golangci-lint run` with and without `--build-tags=integration` (0), `go
test -short -race ./...` (65 ok), `deadcode.sh` + `deadcode.sh testonly`
(ok; the new package is reached from `spend`), integration `-p 1 -race`
for `internal/llmusage` + `internal/backends`, and `-run
'Quota|Status|LLMLog'` for `internal/chat`, `internal/llm`,
`internal/handler`. Independent review by a fresh agent during the
build: 4 minor + 5 nit, all fixed or documented in the wave report.
## Not verified
- Clock skew between the daemon and the database shifts the windows by
that skew (the old query used the database clock); in production both
run on one host.
- The planner edge above is observed at ~30 synthetic tenants, not
measured on the live rollup.

…-5) (#75)
Wave W06-5 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §4.7). No migration, no `/v1` surface.
## What this changes
- `/api/status` `llm_24h` (server view, `queryLLM24h`) and the
per-tenant `llm_24h` generation (`buildTenantRollupInto`) read the
hourly rollup `context_llm_usage_hourly` from the first full UTC hour
after `now − 24 h` plus the raw `context_llm_log` rows of that first,
partial hour, through two new readers in `internal/llmusage` (`Groups`,
`TenantGroups`), instead of scanning 24 h of raw rows per tick. The
tenant reader joins both parts through `context_api_keys` →
`context_tenant_scopes` exactly as the raw query did; a consumer's row
(`api_key_id` NULL) counts for no tenant.
- `llm_24h_complete` comes from the rollup's `unattributed_ok`
(successful rows without a backend) plus the same count over the edge
rows: a failed row without a backend keeps `complete` true, as before.
- Output is byte-identical to the raw aggregate (golden below); the one
deliberate difference is a deterministic tie order.
- Docs: `docs/operations.md` (rollup readers now quota + status;
rollback coupling), `docs/architecture.md`.
## Behaviour changes (intended)
- Rows with the same `calls` are now ordered by `backend`, then
`pipeline` (the raw query ordered by `calls` alone and left ties to the
plan; live has 16/16 ties).
- Backend and pipeline names longer than 100 characters show cut to 100
— the rollup's dimension width since migration 159 (the longest live
name has 24 characters). Pinned by a test.
- The window ends at the daemon's clock, not the database's `now()`
(same host in production; tests pin the anchor on the boundaries).
- On an error in the middle of the result the server view returns an
empty list and `complete=false` instead of the rows read so far plus
`false`.

Per status tick: the edge is at most one hour of raw rows bounded on
both sides (23 rows in the plan test vs 1 079 for the raw 24 h scan),
the rest through `idx_llm_usage_bucket`; pinned under the custom and the
forced-generic plan. Tenant view: the same two parts joined to the key
and scope tables.
## Verification
Gates, each red on `14b99639` first: (a) existing suites green before

JSON of both views generated on the base tree from the base SQL (only
`now()` → `$1` and the tie-break added; the base functions match it up
to tie order), new code byte-equal (same sha256, empty diff); red on
base by compile error, red against overlays: no edge hour 253/300 seeds,
truncated mean 262/300, no tie-break 298/300; (c) `complete` probe: red
when the rollup read omits `unattributed_ok` and when the edge does not
count it; an error row without a backend keeps `complete` true; (d)
`EXPLAIN` custom + generic: 23 raw rows (exactly the edge) vs 1 079,
rollup via `idx_llm_usage_bucket`; the base SQL fails the same check;
(e) consumer rows absent from the tenant view — pinned, red against an
overlay that credits them to a tenant.

`internal/backends`. Independent review by a fresh agent during the
build: 1 minor (the 100-character cut — documented and pinned), 4 nits
(3 fixed, the error-path change documented above).
## Not verified
- Clock skew between daemon and database shifts the window by that skew
(same host in production).

…r and channel (Migration 160) (#76)
Wave W06-6 of the OpenAI-compatible API plan (RC-A, telemetry
foundation; design/06 §3.3 M-06-3, masterplan K16). Migration **160**,
no new table, no `/v1` surface. The read surface `GET /api/audit`
follows in W06-7.
## What this changes
- `go/migrations/160_audit_keys_quota.sql`: audit rows in
`context_settings_audit` for `context_api_keys` (`create`, `update`,
`revoke`, `reactivate`, `delete`) and `context_tenant_quota` (`create`,
`update`, `delete`). State is projected through **whitelist** functions
`audit_state_api_key` / `audit_state_tenant_quota` (never `to_jsonb(NEW)
- …`): `key_hash`, `last_used_at`, `created_at`, `updated_at`, `id`
never land in `old_value`/`new_value`. The key UPDATE trigger fires only
on the ten audited columns and only when one of them changed, so the
per-request `last_used_at` write of authentication never audits
(B-06-1). `audit_actor_meta()` reads the transaction-local stamps
(`ctx.api_key_id`, `ctx.request_id`, `ctx.audit_via`) and resolves the
actor's label, principal, tenant and server-admin flag into `metadata` —
the inputs of the W06-7 redaction. Backfill: one `snapshot` row per
existing key and quota row with `metadata.tenant_id` (12 rows on the
live install), idempotent. Index `idx_settings_audit_entity
(entity_type, created_at DESC)`.
- `internal/store/audit_via.go`: the `via` vocabulary as Go constants
(`api`, `bootstrap`, `provision`, `offboarding`, `migration`; `sql` = no
stamp), `AuditStamp`, `stampAudit` (one `set_config(…, true)` statement
inside the write transaction). Every API-driven writer of the two tables
now stamps actor + request id + via: `MintKeyWithQuota`, `MintOwnerKey`
(tenant create, project provision), `DeleteApiKey`, `UpdateApiKey`,
`UpsertTenantQuota`, the bootstrap admin key (now in a transaction,
`via='bootstrap'`, no actor), tenant offboarding (`via='offboarding'`).
A row written by psql reads `via='sql'` with no actor — the signal of a
direct intervention.
- Classification pin: `audit_classification_test.go` compares the live
column list of each audited table against audited ∪ explicitly excluded
(as data); a new unclassified column turns it red (B-06-2), per table
and per column.
- Docs: `docs/security.md` "Audit trail" (what is audited, what never
is, `via` values, actor fields), `docs/operations.md` "Migration 160".
- Hardening after review (`258e0953`): `stampAudit` refuses a non-empty
stamp on a handle that is not a `pgx.Tx` (`store: audit stamp outside a
transaction`) before anything is written — on a pool the `set_config(…,
true)` would be its own transaction and the write would silently read
`via='sql'` without an actor (red-proof: `MintOwnerKey(pool, APIStamp)`
created the key with a `via='sql'` row on `7d4adf3d`; now an error and 0
rows). Every production caller passes a transaction.
- The private plan submodule pointer is bumped in a separate commit
(plan corpus of this build).
## Behaviour changes (intended)
- Key and quota writes now leave one audit row each; the audit table
grows with the lifecycle of keys and quotas (rare), never with requests.
- The bootstrap admin key is inserted inside a transaction (was a single
statement).
- `UpsertTenantQuota` is stamped as well (otherwise the quota row would
carry no `request_id`).
## Deviations from design/06 §3.3, with reasons
- `via` falls back to `api` only when an actor was stamped, else `sql`
(the design's `COALESCE` would read a psql quota update as `api`).
- Quota trigger: the `updated_by` fallback actor applies on INSERT or
when `updated_by` changed; an UPDATE that changes no audited state
writes no row (no empty rows from `ON DELETE SET NULL`).
- Quota `entity_key` is the `scope` (the row's natural key).
- The index is created before the triggers (shorter lock window,
reviewer finding).
- Two existing tenant-create tests used `"non-admin-key-id"` as an actor
id; the stamp now casts it to `uuid`, so the fixtures use a UUID
(production actor ids are always UUIDs).
## Consumers of the changed contracts

## Overlap search
Open PRs #55/#57/#63/#67 on 2026-11-03: no shared files. Based on #75
(`b4d430cb`); root unchanged since.
## Cost
Boot: catalog work plus the backfill (12 rows live); `CREATE TRIGGER`
takes SHARE ROW EXCLUSIVE on `context_api_keys` briefly (lock_timeout 3
s). Per key/quota write: one `audit_actor_meta` lookup (one indexed
`SELECT` on `context_api_keys`) and one audit INSERT. Per
authentication: nothing — the `last_used_at` UPDATE is outside the
trigger's column list (gate: 6 authentications + 2 no-op updates ⇒ 0
rows).
## Verification
Gates, each red on `b4d430cb` first: store gates 8 FAIL → 8 PASS,
handler gates 4 FAIL → 4 PASS, `via` pin compile error → PASS. Negative
1 (trigger without the column list): 8 audit rows for 6 authentications
+ 2 no-op updates → 0. Negative 2 (blacklist `to_jsonb(k) -
'key_hash'`): `last_used_at`, `created_at`, `updated_at`, `id` leak into
old/new → only the ten whitelisted columns. Negative 3 (probe column
`secret_probe`): classification pin red → green. Negative 4
(`MintOwnerKey` without stamp): `via='sql'`, no actor → `via='api'` with
actor and `request_id`; with all stamps disabled every writer gate was
red, bootstrap, provision and offboarding included. Backfill: N keys ⇒ N
`snapshot` rows with `tenant_id`, second run 0 rows and byte-identical
catalog.
Suites on the head: `go build ./...`, `go vet -tags=integration ./...`,
`golangci-lint run` with and without `--build-tags=integration` (0), `go
test -short -race ./...` (66 ok), `deadcode.sh` + `deadcode.sh
testonly`, integration `-p 1`: `internal/store` (330),
`internal/handler` (138 in the
manage/tenant/key/audit/settings/provision selection), `internal/auth`
(14), `cmd/ctxd` (51), `internal/schemacontract` (44). Independent
reviews: one during the build (0 blocking, 2 should-fix applied — docs
downgrade sentence, per-column pin — 6 notes) and one briefed from the
main context after it (findings folded in below where they changed the
diff).
## Not verified
- The settings trigger writes shadow rows with `via='api'` during
offboarding and `PruneTenant` leaves quota rows in place — both
pre-existing, not touched here.
- `metadata.tenant_id` has no index yet; the W06-7 tenant filter runs on
`idx_settings_audit_entity` plus a JSON predicate (fine at today's row
counts, to be measured at 1 M+ rows).

Thanks for the thorough review. I checked the points against 80251b80735beed793f91469d604feaa73f85f78, read the unchanged brief.go on current origin/root (7c8c6e944669ce986834fb99ad80084b406ae053), and ran focused probes in a detached worktree. No push has been made.
What I reproduced
§3.1 holds, with a scope qualifier. With a project miss, an untagged block whose content starts Project: other. Stack: foreign is injected. Thus len(tags) == 0 is not evidence of project-neutral content. I reproduced the mechanism with httptest; I did not rerun the live-ctxd data census.
§3.2 holds. With 500 newer tagged rows followed by one untagged row, the hook returns 0 bytes on stdout and 0 bytes on stderr after 11 requests (one project lookup plus ten fallback pages).
§3.3 holds. In a synthetic sibling-worktree layout, resolveProject(<sibling>/go) returns <sibling>/go; the main-repository briefing is lost and only the untagged fallback is returned. The equivalent nested worktree resolves to the main repository and keeps its own briefing.
§3.4 holds on the request side. Two untagged rows plus 600 tagged rows at the default limit produce 11 requests and return the two untagged rows. My small fake transferred 78,647 response bytes. I did not rerun the database scans, EXPLAIN ANALYZE, or latency measurements, so those measurements are not part of my verification claim.
§3.5 holds. At --limit 100, 120 project-tagged rows produce 50 blocks, while 120 untagged rows on the fallback path produce 100.
§3.6 holds. A failed project request silently returns only the general block after two requests. A failed second fallback page returns the partial (1 blocks) result after three requests. Stderr is empty in both cases.
§3.7 holds as a test-coverage finding. The existing ^TestBrief suite stays green independently with each of M2 (drop after_updated), M3 (drop compact:false), and M4 (return nil instead of preserving the partial result). The unmodified code is not implicated by these mutation runs; the finding is that the fake does not catch the mutations.
The CodeRabbit overlap wording also holds. A block tagged both /srv/mine and /srv/other is returned for /srv/mine; “tagged only for another project” is the accurate wording.
I also accept the two documentation corrections in §4: the Hermes category includes the MEMORY.md mirror as well as session digests, and the provenance tags come from config.py:328-329, not the cited category lines.
Checks run locally
go test -short -race -count=1 -run '^(TestBrief|TestFormatBrief|TestReviewProbe)' ./internal/cli/ — exit 0.
go test -short -race -count=1 -timeout=10m ./... in a plain clone at 80251b80 — exit 0.
go test -short -count=1 -run '^TestBrief' ./internal/cli/ after each of M2, M3, and M4 — exit 0 for each mutation; brief.go was restored byte-for-byte afterwards.
I did not run the integration shards locally. A fresh GitHub read shows the head checks passing, including those shards.
Fallback key and the pending rework
The rework is waiting for your direction in #58, as you requested in §7.1. The alternatives I see are:
An explicit producer-set marker such as briefing:global. The existing positive tag filter can select it server-side: at most two requests, no 500-row fallback window, no fallback paging cost, and the same server limit ceiling on both paths. It also makes global intent explicit rather than inferring it from missing metadata. The cost is a reserved-tag convention plus retagging existing general briefings and documenting/producing the marker. The decision should identify that producer.
Keep “untagged means project-neutral.” This preserves existing untagged data without migration, but it also preserves the semantic hole demonstrated by §3.1. With the current client filter it retains the bounded window and request cost; it therefore still needs an exhausted-window signal. A server-side untagged filter would remove the window, paging cost, and limit asymmetry, but introduces the REST/MCP parity work discussed in the review. Error handling, worktree resolution, and the test-fake gaps remain independent fixes either way.
The #58 decision should also settle whether topic-only and project:<name> tags are intentionally outside the fallback. Once the key is chosen, the independent items are the .git-file resolution, error/empty separation, the limit rule if it still applies, the stricter fake and failed-page coverage, the CodeRabbit wording, and updating the two #55 sentences in whichever PR lands second.
Direction for the fallback key is settled in #58 (marker tag briefing:global, one request through the existing scope-aware tags filter). The rework list is in that comment; the review above stays "changes requested" until the head moves. Note #55 landed first (31344385), so this PR carries the two README/agent-memory sentences.
Fall back to the producer-set briefing:global marker through one scope-aware search when the project lookup is empty or fails. Keep errors distinct from legitimate empty results and apply the same REST limit to both paths. Update the producer documentation and track broader worktree identity in #146.

Not read: Claude Code's hook runner, web/. Body vs head: matches 933de43f. Every behavioural claim was reproduced (§6); the "RED before implementation" in the body is a compile failure, the mutants in §2/§4.2 add the behavioural red.
1. What the change does
As decided in #58: runBrief looks up the project tag first and, on an empty result or a failed request, sends one more /api/search with tags: ["briefing:global"] (brief.go:111-117). The client-side untagged walk, its 500-row window and both constants are gone. searchBriefBlocks/searchBriefPage now return (result, error), and an unsuccessful envelope, a decode failure and a transport failure are errors (:153-177); briefSearchLimit sends 10 for non-positive and at most 50 (:141-149). The Long text, docs/operations.md:88, README.md:105 and docs/ctx-as-agent-memory.md:30 describe the marker. resolveProject is unchanged; .git-file identity moved to #146. Intent and effect agree on every path I ran.
2. Round-1 findings on this head
Round 1
Status
Evidence
Red proof (test red against "old behaviour")
§3.1 untagged ≠ project-neutral (Medium)
closed[executed]
End to end on PG18, key A, project miss, limit 20: merge base → 20 blocks, the 20 newest rows tagged for another project (/srv/noise); this head → 3 blocks, exactly the marked ones readable by the key. Untagged, topic-only (topic:ops), project:mine and other-project blocks are not injected.
Same probe, marked blocks older than 600 newer tagged rows: round-1 head → 11 requests, 0 bytes; this head → 2 requests, the marked blocks. The filter is server side (tags && $n before ORDER BY … LIMIT, store/blocks.go:1641-1645, :1732-1737).
Structural: no paging code remains; TestBriefFallbackUsesOneRequest pins 2 requests; mutant "untagged-only" turns it red.
§3.3 sibling worktree loses its own briefing (Medium)
resolveProject byte-identical (brief.go:201-214). Probe: resolveProject(<root>/main-feature/go) with .git = gitdir: <root>/main/.git/worktrees/feat does not return <root>/main. On a project miss the head serves only marked blocks, so the repository's own briefing still does not reach a sibling worktree. As agreed in #58; the docs now say "current resolver … tracked in #146" instead of stating the rule as the contract.
n/a (deferred). #146 carries the resolver part; see §4.5 for two items it does not name yet.
§3.4 up to 11 full-content requests (Low)
closed[executed]
2 requests at most on every probed case (base 2, round-1 head up to 11).
Mutant "no client clamp" → TestBriefOutgoingLimitUsesClampAndDefault red.
§3.6 error = empty (Low)
closed as decided[executed]
The helper separates them (TestBriefSearchSeparatesEmptyAndError); a failed project request reaches the marker fallback (3 tests); both failing → no output, exit 0. runBrief discards the error at both call sites, so the two cases stay indistinguishable for the user — that is the "degrade silently" contract of docs/cli.md:23 and what README now documents ("returns nothing or fails").
The fake honours compact (default true, fails the test on compact:true), serves 500 {"success":false}, cuts the transport and returns malformed JSON; the cursor rule is moot without paging.
git grep 'whatever their tags' -- README.md docs/: root 2 files, this head 0, merged tree 0. Both sentences now name the marker and the failure case correctly.

CodeRabbit "tagged only for another project"
adopted[read]
Long text brief.go:68-69 and docs/operations.md:88. Thread discussion_r4448599472 is outdated and still unresolved.

3. Overlap and duplicates
Search scope: open PRs and issues of GottZ/ctx (1 open PR, 6 open issues), gh search prs and gh search issues without state filter for briefing:global, briefGlobalTag, resolveProject, commondir, gitdir, brief fallback, agent-briefing, HashNOOPCheck; git merge-tree against current root.
Same package, no shared lines; tree merged with root builds and tests green.
4. Findings (ranked) — new in this round
4.1 Low — The MCP store tool cannot retag an existing general briefing with the marker while #139 is open; the docs call it equivalent [executed]
Category: claims
Where:docs/operations.md:88 ("or the equivalent MCP store tag"); cause in handler/mcp.go:561 (store.HashNOOPCheck, content only, #139)
Entry point: An owner follows #58 ("Existing general briefings are tagged once by their owner") through the MCP store tool: same category, title and content, tags: ["briefing:global"].
Before → after: Not a regression; new relevance. PG18 probe: MCP → No change needed — identical content already exists (id: …), tags afterwards []; the same sequence through POST /api/store → "action":"updated", tags ["briefing:global"]. For a new block the MCP path works.
Who pays: Owners who retag through MCP (the agent-facing door). The briefing stays out of the fallback; the tool answer reads like success.
Fix: One clause in docs/operations.md:88: existing blocks are marked with ctx save … --tag briefing:global (same content is a real write since #134) or ctx manage update <id> '{"tags":[…]}' (replaces the whole tag array); or leave the doc and add this sentence to #139's scope so it is re-checked when #139 lands.
4.2 Low — No test pins the literal briefing:global; changing the constant keeps all tests green [executed]
Category: tests
Where:brief.go:18; every fixture uses briefGlobalTag (brief_fallback_test.go:152, :174-175, :191, :238, …)
Entry point:go test -short -run TestBrief ./internal/cli/ after editing the constant.
Before → after: Mutant briefGlobalTag = "Briefing:Global" → all 9 TestBrief* pass. Tag matching is exact and case-sensitive (tags && $n; probe: Briefing:Global and briefing:global are not selected), and producers type the string by hand from the docs, so a drift would empty every fallback without a red test.
Fix: Use the literal "briefing:global" in one fixture (for example b4 in TestBriefFallbackRequiresGlobalTag), or if briefGlobalTag != "briefing:global" { t.Fatal(…) }.
4.3 Note — briefSearchLimit repeats the server's own rule [executed]
handler/context_search.go:149-151 (default 10, cap 50) and store.ClampLimit(…, 10, 50) already apply it. On the real handler the merge base, which sends the raw --limit (0, −5, 51, 100), returns the same block counts as this head (10, 10, 50, 50). The clamp changes the wire value only. The rework list allowed it, so this is your call; if it stays, briefMaxLimit's comment could say it mirrors the handler cap rather than defining it.
4.4 Note — The helper's error has no reader in production [read]
Both production calls discard it (brief.go:113, :116), and the briefResponse.Error message branch (:32, :171-173) is read by no test (they check err != nil only). This matches the decision ("separated … in the search helper") and the silent-hook contract. If a consumer is planned, one stderr line on a failed project lookup is the smallest one; otherwise the message branch can collapse into the generic error.
4.5 Note — #146 carries the resolver, not yet the second consumer [read]
ctx persist --hook tags its writes with the same resolveProject (persist.go:95). Once #146 changes the identity, blocks already persisted from sibling checkouts carry the old path and stop matching. #146 says "consistent tags for the main checkout and linked worktrees" but names neither persist.go nor a hook-path test through stdin (projectFromStdin). Worth one line on #146; nothing to change here.
4.6 Note — ctx brief --help cites an issue number [read]
brief.go:69-70 ("Broader .git-file worktree resolution is tracked in #146.") is the only issue reference in a CLI help text (grep over internal/cli), and the help text does not state the .git-directory rule it qualifies. The sentence can go; docs/operations.md:88 carries the pointer.
5. Declared behaviour changes (not defects)
Marker-only fallback, live consequence. Read-only census on the maintainer's store: 7 agent-briefing blocks, 0 carry briefing:global, 2 untagged. After deployment every project without its own path-tagged briefing gets no briefing on that instance until an owner marks general briefings (and per round-1 §3.1 the two untagged ones are other projects' briefings, so they should stay unmarked). Intended by #58.
Granted blocks take part. A marked block in another tenant's scope that is granted to the key's tenant is injected (probe: B granted global); the same block without the grant is not (B foreign global). This is the "read scope and block-grant clause apply unchanged" of #58.
A failed project lookup serves the global briefings instead of the project's own; documented in README and the Long text.
Exact spelling.Briefing:Global and a leading space are not the marker.
Without a resolvable project (os.Getwd fails) only the marker request is sent.
6. Checked and found in order
Body claims, each reproduced: one server-side fallback request (2 requests total on every miss); untagged / topic-only / other-project blocks not injected; project match wins (1 request, global not merged; TestBriefProjectBlocksWin, mutant "always merge global" → 3 tests red); errors distinct in the helper and still reaching the fallback; both failing → silent, exit 0 (also with an invalid key against the real handler.Auth: 0 bytes); limit normalisation 0/−5/51/50/100; ctx brief without --hook takes the same lookup (human and hook output probed in every case).
Trust boundary: the marker request goes through searchBlocksSQL with (scope = ANY($1) OR id = ANY(grants)) AND-ed to tags && $n (store/blocks.go:1641-1645, :1697); search_core.go:194-200 passes the key's read scopes and resolved grants. Probe: a marked block and a /srv/mine-tagged block in a scope the key cannot read are never injected.
Hook path through stdin (projectFromStdin → resolveProject → project request) end to end: the .git-directory repository's block is served with 1 request on base and head.
Caller sweep: searchBriefBlocks, searchBriefPage, briefSearchLimit, briefGlobalTag, briefMaxLimit are referenced only in brief.go and brief_fallback_test.go; runBrief only by briefCmd and tests; the (result, error) change has two production call sites, both in runBrief. resolveProject unchanged, second consumer persist.go:95.
The documented producer command ctx save agent-briefing <title> --tag briefing:global (flag after the positional arguments, content on stdin) sends tags: ["briefing:global"].
Cost: at most 2 sequential requests per hook run, as on the base; worst-case wall time unchanged (2 × client timeout).
Docs against code: README and agent-memory sentences (failure classes "transport error, malformed or unsuccessful response" match searchBriefPage), docs/operations.md:88 (resolver description matches brief.go:201-214; limit sentence matches §2).
Scope of the diff: every hunk maps to a #58 rework item or a round-1 finding (dossier has the table); no API, store or migration change.
Commits: all three SSH-signed, verified=true, author = committer. The history keeps the round-1 commit (paging code) and a merge from root; a squash merge keeps root's history to one commit.
7. Existing review coverage
Round 1 (GottZ, 2026-11-02, review 5744885912): status of every item in §2.
Your reply (2026-11-03) reproduced all eight round-1 items; the open points it listed for the rework are all addressed on this head.
coderabbitai[bot], review 5786633499 on 933de43f (2026-11-05 14:35Z): no inline findings; summary comment 6589724135 (updated 14:39Z): merge risk "Minimal", security architecture risk "Minimal", one pre-merge warning "Docstring Coverage 40.00 % < 80.00 %". Not adopted: the repository has no docstring gate (lint 0 issues), and the undocumented touched functions (runBrief, briefCmd) were undocumented on the base.
New in this round: §4.1–§4.6.
8. Verdict
Approve. All three round-1 Medium findings are settled on this head as agreed in #58 — §3.1 and §3.2 closed with red-to-green evidence against the real handler, §3.3 tracked in #146 — and so are the five Low items; CI is green and the full short suite passes with and without -race. The two Low items below are small and can land here or as a follow-up. Open gates: the integration shards were not run locally (green in CI), and there was no run against a live ctxd or a real Claude Code hook.
§4.1: name the retag path for existing blocks in docs/operations.md:88, or route the sentence into #139.
§4.2: pin the literal briefing:global in one test.
Optional: §4.3, §4.4, §4.6; one line on #146 for §4.5; squash on merge.
9. Not verified
Integration shards (handler, store, guard, heavy, distill, rest) not run locally; green in CI on this head. My PG18 probes ran only the cases listed above.
No run against a live ctxd and no real Claude Code SubagentStart hook; the live store was only counted read-only.
Cross-compiles (windows/amd64, darwin/arm64) not run locally; green in CI.
The .git-file layouts beyond the linked sibling worktree (bare, submodule, --separate-git-dir) were not probed; they belong to #146.

What this changes
Reworks #63 according to the maintainer decision in #58:
ctx briefandctx brief --hooknow fall back through one server-side search foragent-briefingblocks taggedbriefing:global..git-file worktree identity is tracked in cli: canonicalize ctx brief project identity for .git-file worktrees #146; this PR deliberately keeps the existing resolver semantics.docs/operations.md,README.mdanddocs/ctx-as-agent-memory.mddescribe the producer-set marker and the follow-up boundary. The previous client-side untagged pagination, its 500-row window and its request asymmetry are removed. No API or producer migration is included; owners tag existing general briefings once as documented in #58.
Fixes #58.

ctx brief --hooknow uses only blocks taggedbriefing:globalwhen the project-specific search returns no results or fails. Project-matched blocks take precedence; untagged and differently tagged blocks are excluded from fallback results.
